Youth Mental Health and Addictions (MHA) Navigation and Transition Program
About the Youth MHA Navigation and Transition Program:
The Youth MHA Navigation and Transition Program is a collaboration of youth mental health and addiction service community partners, Regional Ontario Health Teams, patients and caregivers with the goal of improving patient experiences by designing seamless transitional care for youth ages 16-28 with mental health conditions and addictions. The committees are looking to expand navigation and transition services in York Region for children and adolescents addiction services.
We are looking for individuals 16-28 years old with lived experience accessing mental health and addiction services who are:
Available to actively attend and participate in meetings (Steering Committee meets every quarter and Operations Committee meets once a month) and completes assigned activities. All meetings are conducted virtually.
Able to work with a diverse group of individuals including youth, caregivers and community partners.
As a member of the Youth MHA Transition and Navigation Operations Committee, you will:
Follow Healthcare Navigation Service (HCNS) terms of navigation, care coordination and warm transfer
Incorporate the patient, family and caregiver Declaration of Values
Refine referral pathways and create a continuum of care that ensures a seamless transition
Develop performance indicators to measure program outcomes to inform continuous quality improvement
Ensure patients, clients and caregivers are meaningfully involved with planning
