Our team

The EYRND OHT began its journey with a task force composed of leaders from partner organizations. Together we set out to create a strategy for the OHT and the development of the application. All partner organizations have signed the EYRND OHT collaborative agreement and through an open, transparent election process have chosen representatives from acute care, primary care, long-term care, home and community services, non-health sector, municipalities of York Region and Durham Region as the EYRND OHT Core Leadership Council.

A Performance Management Committee composed of representatives from acute care, home and community care and York region has been created to monitor the overall project metrics, funding and performance.

Our Primary Care Advisory Council engages with local primary care providers and offers updates to Markham Stouffville Hospital’s internal clinical teams. Representatives with lived experience, and caregivers are also valued members of OHT team.
