Streamlined Access

Streamlined Access began in 2007 as a partnership between several community mental health and addictions services, including Your Support Services Network, Canadian Mental Health Association, Loft Crosslinks, and Addiction Services Central Ontario to provide a coordinated access for case management services in York Region and South Simcoe.

Since 2007, Streamlined Access has expanded to become the access mechanism to the Assertive Community Treatment Teams, Psychogeriatric Assertive Community Treatment Teams, and Mental Health and Addictions Supportive Housing.

Streamlined Access also provides access to a number of mental health and supportive housing programs to individuals 16 years of age or older.

Most recently, Streamlined Access has developed a referral pathway specifically for primary care providers.

The purpose of the primary care referral pathway is to connect patients to community mental health and addictions services, through Streamlined Access. The referral pathway was launched to a small number of primary care providers in July 2021 and will continue to be rolled out to primary care providers, with full onboarding in the winter of 2022.

To support primary care providers, Streamlined Access will complete:

  • intake assessments,

  • waitlist management for the 26 (and growing) programs offered through Streamlined Access

  • offer both peer and family caregiver support, and navigation to services that are not available through Streamlined Access
