What is an OHT?

The Government of Ontario is reforming the health care system by creating Ontario Health Teams (OHTs), established by the new Connecting Care Act. The Act also creates Ontario Health, a ‘super’ agency that provides broad oversight to the health-care system.

OHTs are groups of health care providers who will deliver care to people with lived experience including primary care services, home care, acute care, long-term care, mental health and addictions services, dementia support and care, and palliative care services. OHTs will allow care providers to be more efficient, and spend more of their time and resources delivering care to clients.

Our EYRND OHT has strong support from local primary care providers. EYRND OHT is excited to have the involvement of health care providers as it takes this collaborative journey together to provide seamless care to residents.
