Message from our Co-Chairs

Over the past year, EYRND OHT has made significant strides in fulfilling our mission to provide patient-centered coordinated care that is accessible, efficient, and of the highest quality. We have tackled various challenges head-on, working collaboratively with our partners and stakeholders to enhance the health and well-being of our community.

In this report, you will find more information about our successes and accomplishments. We extend our heartfelt gratitude to all our partners, health care providers, staff, and community members who have contributed to our achievements this year. Your dedication and tireless efforts have made a tangible difference in the lives of those we serve.

Access to primary care

In 2023, we opened two clinics to help make primary care more accessible.

In partnership with Oak Valley Health and Well Plus Compounding Pharmacy, we opened the Community Health Clinic (CHC) located in Stouffville. The CHC is a nurse practitioner-led clinic that offers same-day and next-day appointments for those who are six months of age or older.

In addition to the CHC, we partnered with Oak Valley Health and Ontario Health to open the Central Region Virtual Urgent Care Clinic (VUCC). This clinic operates after hours and is available for those who require timely access to care for urgent, non-life-threatening conditions that can be addressed virtually.


Navigation and care integration

In 2023, we continue to work towards providing a seamless experience for those who are seeking care. In March, we successfully implemented a consistent model for navigation calls to six organizations across York Region.

This is an improved process for the navigators of the participating organizations that will link callers seamlessly to appropriate services that meet their needs and provide a standardized experience regardless of which organization is contacted.


Physician engagement

The 2nd Annual EYRND OHT Town Hall took place on September 26, 2023, with the goal to continue strengthening communications between EYRND OHT and primary care physicians in order to foster long-term connected care.

In-person town hall provides opportunities for networking for over 115 members, a platform for information sharing of strategic priorities of EYRND OHT, and discuss ways to meet the needs of patients and providers and improve access to care in the community.

A look back at 2022-2023

Acknowledgements: We would like to acknowledge our partners, volunteers, including students and persons with lived experience who have helped shape the evolution of the EYRND OHT by dedicating their time and efforts to ensure their voices were heard. Their contributions helped us improve the patient and caregiver experience by connecting them to the appropriate services seamlessly and effectively.
