2024-2025 annual report: Advancing Integrated Care
Message from our Co-Chairs
As Co-Chairs of the Eastern York Region North Durham Ontario Health Team, we are proud to present the 2024–2025 Annual Report, showcasing our shared progress toward accessible, coordinated, and high-quality care for residents of Markham, Stouffville, Uxbridge, and beyond.
This annual report highlights our collective accomplishments and the unwavering dedication of our partners, providers, staff, and community members who continue to make a meaningful impact. As we advance integrated care, we remain committed to innovation, collaboration, and improving health services to meet the evolving needs of our community.

Every time Jeannette Valdez took her mother, Carolina, to a medical appointment, it was difficult.
Jeannette, who was her 94-year-old mother’s sole caregiver, frequently had to arrange transportation and hire a support worker to help. “When I took her to the hospital for an appointment it was in an ambulance, not a car, because one time she almost fell,” explains Jeannette. “It was always a big challenge.”
What happens when patients call 911, but after paramedics complete their assessments, they determine that the most appropriate place for care is a primary care provider rather than the Emergency Department?
EYRND OHT and the York Region Paramedic Services are co-designing care pathways so that when patients are discharged at home, they’re referred and connected directly to their family doctor, a nurse practitioner at Community Health Clinic Stouffville, or the Virtual Urgent Care Clinic.

Building on the success of our nurse practitioner-led Community Health Clinic Stouffville, Children’s Rapid Access Clinic, and the Virtual Urgent Care Clinic, we launched a new clinic in Uxbridge in October 2024.
In just a few months after launch, Shahnaz Kamali Anadani, Nurse Practitioner, Community Health Clinic Uxbridge, had 1,277 visits, demonstrating strong community need.
